  1. We provide services for the entire family, newborns to grandparents. Children less than 18 years old need to be accompanied by a parent or written parental permission for us to treat the child must be given to the adult accompanying the child.
  2. When scheduling the appointment, please inform the receptionist of the reason for your appointment so that proper time can be allotted in the schedule.
  3. Please present your insurance card and a picture ID at each appointment. All co-pays, deductibles, and payment for non-covered services are due at the time of service. Be sure to notify us of any changes in your name, address, phone numbers or insurance coverage.
  4. If you are unable to keep your appointment, please notify us 24 hours in advance or you will be charged for the appointment time we have reserved for you.
  5. We notify you of all lab results either by mail or phone. We will contact you within 2 weeks with the results. Please do not call before 2 weeks. Pap results take up to 3 weeks. If you have not heard from us after 2 weeks for lab results and one month for a pap smear, please call us (703) 379-8879.
  6. For billing or insurance questions, call the billing department (703) 379-8879, ext. 194. Please leave your name, phone number, insurance and a brief message. A patient representative will research your account and return your call. For inquiries about your covered benefits please call the Member Services phone number listed on your insurance card.
